_____	Copy of Passport ID Page
_____	Copy of Passport US Visa Page (not applicable is I-94 indicates VWB/VWT visa waiver)
_____	Copy of I-94 (Arrival/Departure Card stapled in passport)
_____	Copies of I-20’s-US Department of Justice Immigration and Naturalization Service “Certificate of Eligibility for Nonimmigrant (F-1) Student Status”
_____	OR Copies of DS2019- US Department of State “Certificate of Eligibility for Exchange Visitor (J-1) Status”
